To sign up for R4PDP trainings or services, please use the forms below.  Read directions carefully, observe deadlines, and be sure to submit all required information or materials.

The forms are set up so you can fill them out on your computer using Adobe Reader.  If your computer automatically opens the form in an internet browser window, be sure to save the file as a PDF document and open it with Adobe Reader before filling it out.  The forms work best with the most recent version of the Adobe Reader software.  You can download Adobe Reader for free using the link below.

When you fill out the forms on your computer you can save them with your information and then email them to us at R4PDP@ccrcinc.org.  Or you can print them and return them by regular mail to:

Use the form required for the type of activity you are interested in. 

• R4PDP TRAINING APPLICATION FORM to sign up for CEU trainings listed on the R4PDP Calendar, fill out the application form and return with appropriate payment at least 7 days before the training begins.  You can mail forms with a check or money order or email forms with credit card information.  

• R4PDP COLLEGE COURSE APPLICATION FORM to apply for college courses or to express interest in upcoming college offerings.  NOTE:  We have limited funds for college courses.  This form is an application only.  You will be notified if you are selected to participate in available courses and we will work with you to try to help you find the training you need.

• R4PDP PROFESSIONAL SUPPORT REQUEST FORM to request career counseling, coaching, or technical assistance related to professional development, accreditation, or QRIS for individual educators or for programs.

Profile and Planning Forms 

For Educators creating an Individual Professional Development Plan

• R4PDP Educator Profile & Individual Professional Development Plan (IPDP) Use this form for setting your long and short term goals and beginning your planning process.  You can use this form on your own, or share it with your supervisor, mentor, or coach.

For Providers creating a Program Quality Improvement Plan

• R4PDP Provider Profile & Program Quality Improvement Plan (PQIP)  Use this form for setting goals for the development of your program, reviewing your current staff professional qualifications and needs, and focusing your planning with respect to accreditation, QRIS, or other efforts.

Accreditation/CDA Fee Application

• R4PDP CDA or Accreditation Fee Assistance  Use this form for educators applying for CDA or programs applying for accreditation to request assistance with the associated fees.

Directions for completing and emailing forms:

You can work with these forms on your computer PDF files that you can fill out and save with your data so that you can return them by email.  To fill out and email the form follow these directions.

1.     Open the PDF form with Adobe Reader and fill in all the blanks on your computer.  (Do not fill in the form in your browser window or it will not save your additions.) 

2.     Save the completed form to your computer.  Go to FILE menu in Adobe Reader and choose "Save As" >>> "PDF".

3.     Rename the file by adding your last name and first initial or the name of your program at the beginning of the form name; for example "Smith,J_R4PDP_TRAININGAPPLICATION_fillable.pdf" or "BellevilleChildcare_R4PDP_CDAorAccredFeeAssistanceform_FY2012fillable.pdf".

4.     Attach the saved file to an email and send to R4PDP@ccrcinc.org.
